You shouldn't be buying video cards from Best Buy or Circuit City to begin with.
Don't listen to gla300. An 8800 GT can run Fallout 3 perfectly fine. I can play on high settings without a hitch using an HD 3870 on 1280x1024. As  post above me says, typically you shouldn't be buying PC components from a big brick and mortar retailer, as they tend to drastically overcharge. You're almost always better off checking sites like Newegg, Zipzoomfly or Tigerdirect. If ~$120 is your budget, you'd be best off with something like this:http://www.newegg.com/Product/Product.aspx?Item=N82E16814130397It runs cooler, comes from a better brand, you get a lifetime warranty AND step-up program. Plus you get that $20 MIR.  If you can spare a bit more money, you can find some HD 4850's and 9800 GTX+'s for for around $150 to $170.  Even less after MIR's. Heck, there are even some HD 4870's and GTX 260's on Newegg for $200.  Oh, I'm also gonna ask the obvious question. What power supply are you using?
Depends on what resolution your playing at.
All depends on your resolution, I can almost max out Fallout 3 on a 384MB 9600GSO @ 1280x800 so medium settings is well out unless your trying to run on a 24'' or more monitor, I reckon you could almost max at 1440x900 or 1680x1050 (or is that 1650x1080, never remember lol)But personally id go for that EVGA 9800GT linked from newegg, thats a good card at good price with Mail In Rebate.
It will run fine unless you run over a huge resolution ( at or over 1900 x 1200) with max Anti aliasing.
hmm.. i say medium.... i ran it at 95% of max on 1680x1050 res with a 8800gtx (non-OC/etc model) if that helps
